Applicants Age 15 or Under Without Both Parents Present

Adult Passport Renewal (DS-82)
Adult passport holders may renew their 10-year U.S. passports in person or by mail. We reserve the right to request an interview with respect to any passport application. If choosing to apply in person, please schedule an appointment.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
Lost or stolen passports can only be remedied during business hours. Follow the steps in this link to make an appointment and complete your application, making sure to indicate that your passport has been lost or stolen. A police report is not mandatory but is strongly recommended and can help confirm the circumstances of the loss or theft. Do not spend time obtaining a police report if doing so will cause you to miss a flight or delay your travel unreasonably.
Update Current Passport or Replace Emergency Passport
You may be eligible to use this form if you need to change or correct your passport, or you need to replace a passport that was limited to less than 10 years validity (which is the normal validity period for an adult’s passport).
You can fill out the correct form, or find information about your passport status at https://travel.state.gov.
You must provide one photo on a white background with your passport application. Our photo policies apply to adults and children. We have a few exceptions to our photo policies for infants.
Emergency United States passports are issued to applicants who have immediate travel and cannot wait to receive a regular full-validity passport before traveling. Situations in which an emergency passport can be issued include replacement of passports that have been recently lost or stolen and cases of life or death (i.e., serious illnesses, injuries, or deaths in your immediate family) when travel must be undertaken before a regular passport would be processed (which is typically 3-4 weeks).
